Responsibilities
- Provide direct patient care and support during night shifts and weekends
- Assist with patient mobility, vital sign monitoring, and medication administration
- Maintain accurate electronic health records (EHR) documentation
- Collaborate with interdisciplinary teams for seamless patient transitions
- Respond to emergency situations following established protocols
- Ensure patient rooms and equipment are sanitized and stocked
- Support patient education and discharge planning processes
Qualifications
- High school diploma or equivalent (CNA certification preferred)
- Minimum 1 year healthcare experience in acute or long-term care
- Valid CPR/BLS certification
- Ability to stand/walk for extended periods during night shifts
- Strong attention to detail and documentation accuracy
- Exceptional communication and interpersonal skills
- Flexibility to work weekends and rotating night shifts
- Basic knowledge of medical terminology and EHR systems
