Responsibilities
- Coordinate patient care plans and communicate with families during evening/weekend shifts
- Administer medications and monitor vital signs per physician protocols
- Maintain accurate electronic health records (EHR) documentation
- Collaborate with on-call physicians and nursing staff for urgent patient needs
- Support patient education on chronic disease management
- Ensure HIPAA compliance and safety protocols are strictly followed
- Assist with patient admissions and discharge procedures
Qualifications
- High school diploma or equivalent; CNA/LPN certification preferred
- Minimum 1 year healthcare experience (new grads welcome with training)
- Valid CPR/BLS certification (will provide training)
- Exceptional communication and interpersonal skills
- Ability to work independently and prioritize tasks
- Flexibility to work evenings, weekends, and holidays
- Proficiency with EHR systems (training provided)
