Responsibilities
- Conduct outreach and enrollment assistance for Medicaid/CHIP programs
- Provide culturally competent health education to diverse community groups
- Coordinate transportation and resource navigation for vulnerable populations
- Maintain accurate client records and documentation in secure databases
- Collaborate with healthcare providers to address social determinants of health
- Participate in urgent response initiatives during public health crises
Qualifications
- High school diploma or equivalent (Associate's degree preferred)
- Minimum 1 year experience in community outreach or social services
- Fluency in English and Spanish (bilingual candidates strongly encouraged)
- Proficiency with electronic health records and Microsoft Office Suite
- Valid Wisconsin driver's license with reliable transportation
- Ability to work flexible hours including evenings/weekends
- Current CPR/First Aid certification (or willingness to obtain)
