As a Patient Care Coordinator, you'll be the first point of contact for patients requiring evening and weekend services, ensuring seamless care coordination while maintaining our high standards of service excellence.
Responsibilities
- Manage patient intake and scheduling for evening/weekend appointments
- Coordinate with on-call medical staff for urgent patient inquiries
- Update electronic health records with accurate patient information
- Process insurance verifications and billing documentation
- Assist with patient education materials for after-hours care
- Maintain clinic supplies and equipment during weekend shifts
- Provide compassionate support to patients and families during off-hours
Qualifications
- High school diploma or equivalent (Bachelor's preferred)
- 1+ years of healthcare coordination or customer service experience
- Proficiency with EHR systems (Epic or Cerner experience a plus)
- Strong communication and conflict resolution skills
- Ability to work independently with minimal supervision
- Flexibility to work evenings (4pm-12am) and weekends
- CPR/BLS certification (or willingness to obtain)
- Spanish language proficiency highly desirable
