Responsibilities
- Provide direct patient care including medication administration, vital sign monitoring, and personal hygiene assistance
- Document patient progress and care activities accurately in electronic health records
- Collaborate with healthcare teams to develop and implement individualized care plans
- Respond promptly to patient needs and emergency situations during assigned shifts
- Maintain a clean, safe, and organized patient environment
- Assist with patient mobility transfers using proper techniques
- Communicate effectively with patients, families, and healthcare staff
Qualifications
- High school diploma or equivalent required; CNA certification preferred
- Minimum 1 year of healthcare experience in assisted living or hospital setting
- Current CPR/BLS certification and valid driver's license
- Ability to lift up to 50 pounds and stand/walk for extended periods
- Strong communication skills and compassionate bedside manner
- Reliable transportation for flexible evening/weekend scheduling
- Proficiency in electronic health record systems
